A bunion is far more than a cosmetic concern. It is a complex deformity of the big toe joint that typically worsens over time, often leading to pain, difficulty wearing shoes, and changes in how a person walks. The bunion forms when the bones of the front of the foot move out of alignment, causing the big toe to drift toward the second toe and a bony prominence to develop on the inner side of the joint. Bunions develop for many reasons. Genetics plays a major role, and bunions tend to run in families with inherited foot structures that predispose the joint to instability. Years of wearing tight, narrow, or high-heeled shoes accelerate the problem by squeezing the toes and placing unnatural force on the joint. Conditions such as flatfoot, ligament laxity, and certain forms of arthritis can also contribute. Tailor's bunions, which form on the outside of the foot at the base of the little toe, develop through similar mechanisms.

Symptoms typically include a visible bump on the side of the foot, redness or swelling around the joint, soreness that worsens with activity or in tight shoes, stiffness, and the development of corns or calluses where the toes rub together. As the deformity progresses, the second toe may be pushed out of place, creating a hammertoe and further crowding in the front of the foot.

Our evaluation includes a careful examination, gait assessment, and weight-bearing X-rays to measure the angles of the joint and grade the severity of the deformity. Many patients respond well to conservative treatment, which may include wider footwear, padding, toe spacers, custom orthotics to control underlying foot mechanics, anti-inflammatory medications, and night splints. These measures can significantly reduce pain and slow the progression of the deformity, although they cannot reverse the underlying bone alignment.
