The virus that causes plantar warts thrives in warm, moist environments such as locker rooms, public showers, swimming pool decks, and shared bath mats. It enters the skin through small cuts, abrasions, or softened areas. People with weakened immune systems, sweaty feet, or skin that is frequently damp are at higher risk. Once the virus is established, the body produces a thickened layer of skin around the wart, often making it look like a callus and complicating treatment.

Plantar warts have several characteristic features that help distinguish them from corns and calluses. They typically appear as rough, grainy lesions with tiny black dots that represent small clotted blood vessels. The lines of the skin's natural fingerprint pattern bend around the wart rather than crossing through it. Warts are often painful when squeezed from side to side, while calluses are typically painful only with direct pressure. Some patients develop clusters of warts called mosaic warts, which can cover larger areas and are particularly difficult to treat.

Treatment options are numerous because no single approach works for every wart. Topical medications containing salicylic acid or other agents can be effective, especially for smaller, more superficial warts. We may apply medical-strength medications in the office that are stronger than over-the-counter products.  For persistent warts, excisional procedures and other advanced therapies offer additional options.

Treatment often requires patience and persistence, since warts can be slow to resolve and may need multiple sessions. We discuss expectations clearly with each patient and select the approach most likely to succeed for that specific situation. We also educate patients on prevention, including wearing protective footwear in public places, keeping feet dry, avoiding direct contact with other people's warts, and not picking at lesions, which can spread the virus to other areas of the body.
