Severe bunions and tailor's bunions that have failed conservative care are corrected through procedures that realign the bones, often using modern three-dimensional techniques. Rigid hammertoes are surgically straightened, sometimes with permanent internal implants that maintain alignment. Severe flatfoot deformity, especially the adult-acquired type caused by posterior tibial tendon dysfunction, may require reconstruction involving tendon repair or transfer, bone realignment, and joint stabilization. High arches with associated instability may also require surgical correction.

Arthritis of the foot and ankle can be debilitating, and when conservative treatment is no longer effective, surgical options range from joint cleanup procedures and removal of bone spurs to joint fusion, which permanently joins arthritic bones to eliminate painful motion. Total ankle replacement is now an excellent option for many patients with severe ankle arthritis, preserving motion and dramatically improving function. Reconstruction is also frequently needed after trauma. Severe ankle and foot fractures, complex sprains, and crush injuries sometimes heal in poor alignment or fail to heal at all, leaving patients with chronic pain, instability, or deformity. Reconstructive procedures can correct malunions and nonunions, repair damaged ligaments and tendons, and restore the foot to a functional position. Charcot reconstruction is a specialized form of surgery for patients with diabetes who have developed severe foot deformity from neuropathic joint destruction. These complex cases require advanced techniques to stabilize the foot, save the limb, and allow the patient to walk again.
