An ingrown toenail occurs when the edge of a toenail grows into the surrounding skin instead of over it, creating pain, redness, swelling, and frequently infection. The big toe is most often affected, although any toe can develop the problem. While ingrown toenails are common and often dismissed as a minor issue, they can become surprisingly painful and serve as entry points for serious infections, particularly in patients with diabetes or poor circulation. Several factors contribute to ingrown toenails. Improper trimming is one of the most common, especially cutting the nails too short or rounding the corners rather than cutting straight across. Tight shoes, high heels, and pointed-toe footwear squeeze the toes and force the nail into the skin. Trauma such as stubbing the toe or dropping something on it can change the way the nail grows. Genetics plays a role, with some people inheriting nails that are unusually curved or thick. Sweating heavily and certain medical conditions also increase the risk. Once an ingrown nail develops, the body's natural inflammatory response often makes the problem worse, with swelling pushing the skin even more tightly against the nail edge.

Symptoms typically begin with tenderness along the side of the nail, progressing to redness, swelling, and warmth. As the condition advances, the area may produce drainage, develop excess granulation tissue that bleeds easily, and become acutely painful. Even the weight of a bedsheet or the friction of a sock can trigger discomfort. Untreated infections can spread into the deeper tissues of the toe and require more aggressive treatment.

At-home treatment with soaking and antibiotic ointments can sometimes resolve mild cases, but should be avoided by patients with diabetes, neuropathy, or vascular disease. Attempts to dig out the nail at home commonly make the problem worse and introduce bacteria deeper into the tissue. Professional treatment is faster, far more effective, and often eliminates the problem permanently.

In our office, we begin with a careful examination to assess the severity of the ingrown nail and any associated infection. For straightforward cases, we can remove the offending portion of the nail in a comfortable, in-office procedure using local anesthesia. For patients with recurrent ingrown nails, we offer a permanent solution called a matrixectomy, in which the small portion of the nail root that produces the problematic edge is treated so that part of the nail no longer grows. The procedure is highly effective, with success rates exceeding 95 percent, and recovery is typically quick. We also address underlying causes, including footwear choices and proper nail trimming techniques, to keep the problem from returning.
