A hammertoe is a deformity in which one or more of the smaller toes bend abnormally at the middle joint, taking on a shape similar to an upside-down V or the cocked hammer of a firearm. The condition usually develops gradually and worsens over time, eventually leading to pain, corns, calluses, and difficulty wearing shoes. Hammertoes develop when there is an imbalance between the muscles, tendons, and ligaments that normally hold the toe straight. Several factors contribute to this imbalance. Genetics plays a significant role, with certain foot types more prone to developing hammertoes. Tight, narrow, or pointed shoes squeeze the toes into a bent position, and over years this becomes the toe's resting state. High heels worsen the problem by pushing the toes forward into the toe box. Underlying conditions such as bunions, flat feet, arthritis, and nerve damage can all contribute, as can previous trauma to the toe.

Symptoms range from mild to disabling. Early on, the affected toe is flexible and can still be straightened manually, but as the condition progresses the toe becomes rigid and locked in its bent position. Patients commonly develop painful corns on the top of the bent joint where it rubs against the shoe, calluses on the ball of the foot, and sometimes blisters or sores. The toe may overlap or be pushed by the adjacent big toe, especially when a bunion is also present. Walking and even wearing closed shoes can become painful.

Conservative treatment is most effective when the hammertoe is still flexible. Wider, deeper toe-box footwear gives the bent toe more room and reduces friction. Padding, toe sleeves, and silicone caps protect against corns and calluses. Custom orthotics can correct underlying biomechanical issues that contribute to the deformity. Stretching exercises, splinting, and toe straps may help maintain flexibility and slow progression. Periodic professional treatment of the corns and calluses provides ongoing comfort.

Once a hammertoe becomes rigid, conservative measures can manage symptoms but cannot straighten the toe. For patients whose pain or shoe-fitting problems persist, surgical correction provides reliable, long-term relief. Procedures range from soft tissue releases for flexible deformities to bone procedures with internal fixation for rigid hammertoes. Modern techniques use small implants that hold the toe in proper alignment as it heals, often allowing patients to walk in a special shoe immediately after surgery.
